All coaches are required to report, in writing, all serious injuries (sprains, breaks, dislocations, etc..) and/or the need for an ambulance to the both the Vice President of their league and the President of SYSA within 24 hours of the incident. Failure to do this can result in loss of insurance coverage for the injured player and an unfair financial burden to the player/family.
The initial email notification for the coach should include the followinginformation:
Player's name
Player's DOB
Player's team name and division
Coach's name and phone #
Brief description of incident
Following this report, the highest ranking coach present during the incident must complete the NHSA State Injury Report form below and submit it to the President of SYSA within 72 hours of the incident. NHSA requires receipt of the completed form within 1 week of the incident to invoke all insurance rights for the player.
Players/families are then required to submit all copies of bills and expenditures related to the incident according to the processes established by NHSA.
